Passport Services

The Consulate will add up to two sets of additional visa pages to a U.S. passport free of charge.  You may request extra pages in person or by mail. The passport holder must personally sign the application form. A parent must sign if the applicant is a minor under the age of 16. Passports will be available for pickup the next working day, or may be returned by courier or mail/messenger if a prepaid courier envelope is provided. Courier envelopes are available from any western India VFS office for Rs. 166.

Name Changes and Other Amendments

Information contained on the data page of an issued passport, such as name, sex, date of birth, place of birth, and passport validity information, cannot be amended. If information on the passport is incorrect or changes, you must apply for a new passport. There is no fee to issue a new passport to correct or change data if the request is made within one year of the date of issuance of the original passport. To request a free replacement passport to reflect a name change or to change or correct any of the other data contained on the passport’s face page, complete Form DS-5504 "U.S. Passport Re-Application Form."

If more than one year has passed since the passport was issued, you must pay the full renewal fee to obtain a replacement passport. Please follow the instructions for applying for a new passport using Form DS-82 (adult renewals only) or Form DS-11 (other renewals or replacements), as appropriate.
